Abstract

Purpose: Currently, the patients' care in Quebec is not sufficient enough to limit the progression of the knee osteoarthritis (Knee OA) because of the lack of efficient tools and relevant information on the affected population. The « MonArthrose » program (PMA), which is structured around the Knee Kinematic Graph technology (KneeKG), offers such a care plan and, in addition, allows the individualization of the patient's care based on the 3D kinematic analysis of the gait. The purpose of this study was to compare to the usual health care, the impact of the KneeKG technology and the PMA when introduced in primary care for patients with Knee OA. Methods: A first analysis led to the characterization, both on the sociodemographic and the clinic levels, of the Knee OA population in Quebec treated in the sector of primary care. To this end, patients have been enrolled in family practice clinics. Three intervention groups have been constituted: a group which followed the usual health care, a group which had their general practitioner access the results of the KneeKG exam and a group which was taken in charge by the PMA. An exploratory study has also been conducted in order to investigate the existence of kinematic parameters which could possibly be linked to the evolution of pain among patients with Knee OA. Results: The patients who have been taken in charge by the PMA showed significantly improved results compared to usual health care. This has been verified when it comes to their self-reported quality of life (+15 points on the Knee injury and Osteoarthritis Outcome Score, KOOS), to their functional capacities (+3 repeats on the 30 Seconds Chair Stand Test) and to the improvements in their gait kinematic, especially with a diminution of the varus thrust during the loading phase of their gait. The exploratory study has not led to the identification of a kinematic parameter whose evolution would be linked to a reduced pain among patients suffering from Knee OA. Conclusions: The results suggest that the KneeKG, when paired with the PMA, has a significantly positive influence on the patients' daily living comparing to the usual health care. Additional analyses conducted on a larger sample of patients are needed in order to statute on the efficiency of the PMA.
